This blog has been created to assist those charged with caring for individuals and patients across the continuum of care in Yellowstone County and southcentral Montana.

Obviously the most pressing and critical need for those providing care to fellow Montanans is to contain and prevent the spread of coronavirus (COVID-19)

By relying on the most current information available about COVID-19, today the Center for Disease Control and Prevention issued valuable information for healthcare providers.

1. Instructions regarding how to clean and disinfect facilities here.

2. Answers to frequently asked questions about COVID-19, including the use of PPE's to protect healthcare workers and patients alike, may be found here.

Please take a few minutes and familiarize yourelf with this critical information and share it with co-workers, administrators and family members with loved ones under care.

BILLINGS CARE TRANSITONS COALITION

Mission: To improve care transitions and coordination of patient care in the community; improve provider communications and the transfer of essential information during care setting transitions;  and reduce harm, improve medication safety and empower patient-family through education on care, disease, medications and management.

Goals/Objectives:  Improve transitions in care through coordinating care across the community setting to reduce avoidable hospital readmissions. Create a community network of people working together to improve the health and quality of life for the population.
